Introducing
Divine Community Adult Foster Care

Senior Care in Massachusetts

Divine Adult Foster Care is an accredited organization that operates under the laws of Massachusetts and within the local community. Our services cater to individuals aged 16 and above who face various medical, physical, or cognitive challenges. We offer adult foster care in Massachusetts, providing supportive care and assistance to those in need.

What is Adult Foster Care (AFC)?

Adult Foster Care is a program funded by the federal government and administered through MassHealth, aimed at offering living alternatives for those who are elderly or living with disabilities. The program caters to individuals who require assistance with daily activities, both essential and instrumental. Essential activities encompass tasks such as eating, using the restroom, dressing, bathing, transferring, and ambulating. Instrumental activities, on the other hand, are those related to independent living and incidental to one’s care, including household management, laundry, shopping, housekeeping, meal preparation, transportation, and medication monitoring.

How does Adult Foster Care work?

Operating as a licensed Adult Foster Care program, we equip caregivers with specialized training, enabling them to deliver quality care within their homes. Our approach involves collaborating with a team of expert registered nurses and Case Managers. This team conducts monthly home visits to monitor the health needs of our members and clients. Moreover, they develop personalized care plans tailored to each individual’s unique requirements.

Compassionate Senior Care Services

Divine Community Adult Foster Care provides in-home care services for elderly individuals residing in their own homes. Our caregivers possess the necessary skills, knowledge, and compassionate demeanor to deliver dependable assistance, ensuring a fulfilling and healthy lifestyle for our clients. Adapting to independent living can be a daunting transition, especially in the face of natural occurrences such as aging, illness, or disability.
