Our team of specialized professionals remains actively involved throughout the entire journey, from conducting initial assessments to facilitating ongoing home visits, ensuring a seamless and comprehensive care experience.

Our interdisciplinary team includes:

Registered Nurses

We evaluate an individual’s clinical eligibility for our program through the Minimum Data Set Assessment. Our Registered Nurses then create personalized care plans, offering in-home clinical guidance and caregiver education tailored to each participant’s unique needs.

Case Managers

We thoroughly assess home environments to guarantee that members/ clients reside safely under the care of qualified caregivers. Our team advises both caregivers and members, providing essential resources, and verifying adherence to the care plans developed by Registered Nurses.

Community Health Workers

We provide support to Case Managers and Registered Nurses in meeting the monthly visit requirements. Our team may conduct Level I or Level II monthly Adult Foster Care home visits as necessary to ensure comprehensive care and oversight.
